New eligibility - Completed enrollment forms and required dependent documentation must be received within 31 days of eligibility.
Marriage - Completed enrollment forms and copy of marriage certificate must be received within 60 days of marriage. Coverage will be effective on date of marriage.
Birth / Adoption - Completed enrollment forms and copies of birth certificate (with parents names)/adoption papers or court order must be received within 31 days. Coverage is effective upon date of birth/adoption.
Open Enrollment - For any person not meeting the requirements of submission above. Forms must be received within 31 days of effective date of open enrollment.
June - effective date July 1 st.
December - effective date January 1 st.
Divorce Employee should notify the Benefits Department in writing upon the finalization of the divorce (copy of front page of divorce decree preferred). Termination is effective the end of the month in which the divorce is stamped by the Judge.
Employee should notify the Benefits Department in writing when the child ceases to meet the definition of an eligible dependent. Coverage terminates the end of the month in which the dependent no longer is an eligible dependent. Definition of an eligible dependent age 19 for Dental and age 26 for Medical under Obama Care. For Dental only must:
Be a full-time student (proof of full time student status required)
Be dependent upon parent(s) for support and maintenance (copy of employees most recent Federal 1040 noting dependents claimed required)
