Flu Clinic Details

  • All clinics are walk-up only – no appointments needed.
  • Clinics are open to all individuals 6 months and older.
  • Our clinics operate on a first-come, first-served basis and will remain open during the hours listed, or until maximum capacity is reached for the day.

Who Can Receive the Flu Shot

  • Vaccines are available for individuals 6 months and older.
  • BCHD will only administer flu shots to individuals diagnosed with MS or paralysis if a doctor’s note is provided.
  • High-dose flu vaccine will not be available.
  • Pneumonia vaccine is not offered.

Requirements for Children and Teens (Under 18)

Children under 18 must be accompanied by a parent or legal guardian.

  • If a legal guardian brings the child, they must provide a copy of legal guardianship paperwork.
  • If a parent or legal guardian cannot attend, the child’s caregiver must bring:
    1. A completed Parental Permission for Caregiver to Accompany Minor for Flu Vaccination form (PDF)
    2. A copy of the parent/legal guardian’s government-issued photo ID (driver’s license, passport, etc.)
    3. The completed flu vaccination consent form
    4. For legal guardians ONLY – proof of legal guardianship

Note: On the day of the flu clinic, the caregiver must also show their own photo ID.
